End of Life Resuscitation Planning Discussion(s)

Project Description and Aim

  1. To ascertain how many discussions about end of life resuscitation decisions are occurring. This includes both new patients and revisions to patients previous plans.
  2. To record whether the CYPADM ( Children & young people Acute Deterioration Management) form is being used.
  3. If this form is being used, are there any problems with it or changes needed
  4. 4. If it is not being used – what are the reasons for this

Reporting instructions:

Please report any case where discussions occur between medical or nursing professionals and families of children and young persons (CYP) < 18 years of age, about specific planning for resuscitation, in the event of an acute deterioration in a patient’s condition.

Inclusion Criteria

  • CYP ( <16years) with a life limiting illness and where discussion occurs either with the CYP or with the families on behalf of a CYP about planning what resuscitation will take place in the event of an acute deterioration in their condition.

    CYP (16-18years) (who are cared for within paediatric care facilities) with a life limiting illness and where discussion occurs either with the CYP or with the families on behalf of a CYP about planning what resuscitation will take place in the event of an acute deterioration in their condition.
